Medicaid Service Coordination (MSC) is a
Medicaid State Plan service provided by OMRDD which assists
persons with developmental disabilities and mental retardation
in gaining access to necessary services and supports appropriate
to the needs of the individual. MSC is provided by
qualified service coordinators and uses a person centered
planning process in developing, implementing and maintaining an
Individualized Service Plan (ISP) with and for a person with
developmental disabilities or mental retardation. MSC
promotes the concepts of choice, individualized services and
supports, and consumer satisfaction.
MSC consolidates and replaces two earlier
OMRDD service coordination programs, Comprehensive Medicaid Case
Management (CMCM) and Waiver Service Coordination. MSC is
designed to help consumers strive for the highest quality of
life. The focus of MSC is on assisting each person to
achieve his or her unique goals and desires in terms of having a
home, a job, a circle of friends, and enjoyable leisure
activities and access to medical and clinical services.
For individuals with developmental disabilities and their
families, navigating the system of available services is often a
confusing task. Accordingly, Developmental Disabilities
Institute’s service coordination is charged with assisting the
individual and/or their family in defining a person centered
plan with outcomes that lead to successful inclusion into
his/her community as a valued, participating member.
Included in this overall responsibility are:
1.Ability
to obtain supporting information for planning. 2.Help
to identify valued outcomes or personal goals
3.Assist
the individual in the coordination of his individualized
person centered plan;
4.Assuring
that all referrals and associated linkages needed to
achieve the valued outcomes are made;
5.With
the individual, to oversee the Agency/program supports
to the individual ensuring that the person’s interests,
preferences, and capabilities along with inclusion into
the community, are maintained.
In Adult Day
Services, our Waiver Services include Residential and
Day Habilitation services, as well as environmental
modifications and adaptive technologies.
