| The Adult Day
Services Division offers a variety of programs to
developmentally disabled individuals. All individuals reside in
Nassau or Suffolk County either at home or in an
agency-sponsored residence. All programs are based on a
person-centered approach, creating outcomes designed to support
and maintain each adult participant living and working in the
community.
The Adult Day Services Division is divided
into three departments,
Day Treatment,
Family Support
Services, and
Career Development. In all instances it is our belief that a
most important portion of life besides a person’s family for all
individuals, is the time spent working in a meaningful vocation.
It is our goal therefore, not withstanding medical reasons or
people reaching retirement years, that the daytime activity for
the consumers we serve should revolve around productive activity
occurring in the community in which they reside. This belief
becomes the foundation for the programs and supports provided
through the Adult Day Services Division.

Day Treatment
Program |
The Day Treatment Program provides consistent
Monday through Friday, 9:00-3:30 support to individuals with
autism and developmental disabilities who present challenging behaviors.Services
are directed to address interests, preferences and capabilities
of those we
serve as well as improve areas where current skill levels
require it. Members of the person’s family and community are
invited to participate in the person’s plan.
We stress inclusion in community; regular and
routine use of community resources, and training on needed
skills, including those in the job setting through actual
placements and/or training and volunteer sites.

The Day Treatment Program provides a full
compliment of clinical services including psychology,
communication, medicine, nursing, social work, recreation and
psychiatry. Programs are located in Smithtown and Riverhead in
Suffolk County.

Riverhead Day Treatment
Satellite |

The
Riverhead Day Treatment Satellite opened in the fall of 1998 and relocated in
April, 2001 to a larger, more functional facility on Route 58. Twenty-four
adults attend the program, all residing on the east end of Long Island.
Individuals have various living arrangements including family, family care, and
group homes operated by DDI and other agencies.
Our mission is to provide individualized treatment plans
for men and women with developmental disabilities through an interdisciplinary
team model. Providing services based on assessed needs and personal preferences
within or in close proximity to the communities of those we serve has enhanced
their quality of life.
The
professional and caring staff at the site are committed to making a positive
difference in people’s lives. Skill training is provide in Activities of Daily
Living, Independent Living, Respected Roles/Pre Vocational Activities,
Socialization, Life Safety, Communication, Positive Behavioral Support, Language
Enrichment, Recreation & Fitness, Art Therapy, Music Therapy, Monthly Theme
Activities, and Community Integration. Choice, self-management, and skill
enhancement are vital components of each person’s plan.
Participants in our program service the Riverhead area by delivering Meals on
Wheels, Shopping for Senior Citizens, Participating in Island Harvest food
distributions, displaying art work in the community, volunteering at Cornell
Cooperative Extension, and a host of other valued outcomes.

Career
Development |
The highest priority goal for all our adults
is to enable them to hold regular jobs in the community and thus
become productive members of our society. DDI offers a wide
array of employment, work exposure and training opportunities
for the individuals we support. They include Supported
Employment Services, Affirmative Enterprise Training, Employment
Training and Exposure, and Integrated Employment Transportation
Services. Supported Employment places individuals with autism
and developmental disabilities in real job settings within the
community. The necessary supports for success on the job are
provided through an employment specialist who works directly
with individuals with disabilities at the work site. Supports
are designed to meet the specific needs of the individuals and
the work place. Various part time and full time job
opportunities are available at businesses located throughout
Nassau & Suffolk Counties. Our Affirmative Enterprise Training
Services offer work exposure, training and opportunities to many
individuals within our program. These affirmative enterprises,
the CMP Printing & Graphics, the Meadow Glen Greenery (a full
service greenhouse) and Dirt Busters (an office cleaning
business) offer the opportunity to provide exposure to various
jobs, exposure to the idea of work and the associated
responsibilities therein. Our consumers also gain experience by
interacting in a work environment with individuals without
disabilities. Over the course of operation, these enterprises
provide many individuals work experience with competitive
salaries and as a result many have been employed in similar work
opportunities in the business community.
Our Employment Training and Exposure Program
provides a wide array of training and exposure opportunities to
individuals who are entering the work community. Staff are
charged with taking individuals to various sites including the
affirmative enterprise sites and other businesses where we have
developed relationships to offer these individuals the
opportunity to experience the world of work. The overall intent
of this activity is to ensure that individuals have the widest
exposure to different work opportunities in order that they can
make an informed choice as to their preference for work. In
addition, we have developed employment-training classes based on
a curriculum sponsored by the United State Department of
Education and designed by San Diego State University. This
program instructs individuals on employability skills in a step
by step approach. The combination of these classes and our world
of work tours provide a comprehensive basis of information for
the individuals we support.
Finally, many individuals who receive jobs in
the community often have hours of employment that do not fit
into the usual "9 to 5" time frames. Accordingly, through a
grant by Office of Mental Retardation and Developmental
Disabilities, Developmental Disabilities Institute provides
through its Integrated Employment Transportation System,
individualized transportation to and from work for individuals
with disabilities. These services are provided to individuals
with disabilities for all agencies in Nassau & Suffolk County.

Adult
Case Management Services / Medical Service Coordination |
Medicaid Service Coordination (MSC) is a
Medicaid State Plan service provided by OMRDD which assists
persons with developmental disabilities and mental retardation
in gaining access to necessary services and supports appropriate
to the needs of the individual. MSC is provided by qualified
service coordinators and uses a person centered planning process
in developing, implementing and maintaining an Individualized
Service Plan (ISP) with and for a person with developmental
disabilities or mental retardation. MSC promotes the concepts
of choice, individualized services and supports, and consumer
satisfaction.
MSC consolidates and replaces two earlier
OMRDD service coordination programs, Comprehensive Medicaid Case
Management (CMCM) and Waiver Service Coordination. MSC is
designed to help consumers strive for the highest quality of
life. The focus of MSC is on assisting each person to achieve
his or her unique goals and desires in terms of having a home, a
job, a circle of friends, and enjoyable leisure activities and
access to medical and clinical services.
For individuals with developmental
disabilities and their families, navigating the system of
available services is often a confusing task. Accordingly,
Developmental Disabilities Institute’s service coordination is
charged with assisting the individual and/or their family in
defining a person centered plan with outcomes that lead to
successful inclusion into his/her community as a valued,
participating member. Included in this overall
responsibility are:

1. Ability
to obtain supporting information for planning.
2. Help
to identify valued outcomes or personal goals
3. Assist
the individual in the coordination of his individualized person
centered plan;
4. Assuring
that all referrals and associated linkages needed to achieve the
valued outcomes are made;
5. With
the individual, to oversee the Agency/program supports to the
individual ensuring that the person’s interests, preferences,
and capabilities along with inclusion into the community, are
maintained.

Waiver Services |
In Adult Day
Services, our Waiver Services include Residential and Day
Habilitation services, as well as environmental modifications
and adaptive technologies.

Residential
Habilitation Services |
This service of the institute is designed to
provide additional supports to individuals with developmental
disabilities and their families at their homes. These services
are routinely linked to agencies providing day supports for
these individuals. Residential Habilitation Services are based
on our primary focus of supporting each individual in order that
they are a productive and valued member of their community.
Accordingly, social skills involved in the use of community
resources that would normally occur during residential hours,
helping the family during meal time and providing additional
recreational and community resource training are part of our
Residential Habilitation Services. A significant underlying
value of all Residential Habilitation Services is the
recognition that these services are being provided within the
individual’s home and community. Accordingly, the culture and
morays of the particular family supported are those that must be
respected throughout the provision of such services.

Services |

Day Habilitation Services
are age appropriate, meaningful day supports based on each
individual’s capacities, preferences and dreams.
Developmental Disabilities Institute’s Day Habilitation
Services are provided in small groups (no more than 15
individuals). |

The locations for our Day Habilitation
(Day Hab) sites, also known
as Hub sites, are chosen based on the particular
communities from which the consumers in that particular project reside.
Accordingly, all training in the use of community
resources, volunteerism, and activities are based on the
